Adjustement Of Status

Thank you for choosing Innovative Immigration Law to help you with your adjustment of status to Permanent Residence. Please, reference the information below to get started.

Step 1 - Docketwise Intake Form

You will receive an invitation from dockets to complete the candidate intake sheet for your Adjustment of Status procedure. It is essential to fill this out as much as possible.
After attempting to fill out forms – please email adjustment@oaklg.com with the remaining questions. If you do not have any questions, we will email you in case our team needs more information on the forms.

If you have yet to receive an email from docketwise.com within 24 hours, please email info@oandpvisas.com with details. Check your spam folder first if you are still waiting to see the email within 24 hours during the weekday. If it is a weekend, kindly wait until the close of business on Monday.

Step 2 - Fill out Document Checklist Portal

Step 3 - Schedule Visit with Medical Doctor and Bring Form I-693

After we have the form intake filled out, we will send form I-693 for you to take it to a USCIS approved doctor. To find a doctor, click here to schedule an appointment in your area.

1. Form I-693, Report of Medical Examination and Vaccination Record

2. Government-issued photo identification, such as a valid passport or driver’s license. If you are 14 or younger, bring identification showing your name, date, place of birth, and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.

3. Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus, and meningococcal disease)

4. Medical insurance card (check with the doctor’s office to make sure the office accepts your medical plan)

5. Payment (check with the doctor’s office about acceptable forms of payment). Calling a few doctors to determine how much they charge for the exam is also a good idea. Prices can vary by a few hundred dollars.

The medical examination entails a review of your medical history and a physical exam. The doctor will complete a comprehensive assessment, including a review of your medical history and a physical examination. The doctor will also test for infectious diseases such as tuberculosis, syphilis, and gonorrhea.

After the exam, the doctor will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. Ensure you get a copy of the completed Form I-693 for your records before the doctor seals the envelope. USCIS will not accept the form if the envelope has been opened or altered.

For full details, please go to uscis.gov/i-693.

After the exam, you must physically mail the I-693 from the doctor to our office, along with passport photos of yourself and your family members.
Please, submit six passport-style photos of each applicant in the same mailing as form I-693 to the address below:
